BP Solar Settlement Online Claim Form

Please read the following instructions before submitting a claim:

A separate claim form is required for each property where BP solar panels are installed.
This claim form includes several requests for supporting documentation. If your documentation exceeds 20mb, please complete the form without uploading the documents and then email info@BPSolarSettlement.com for information on how to submit the documents separately.
You must currently own the panels you are claiming.
The BP Solar Settlement will send an inspector to evaluate the panels you are claiming.

D. PRIOR WARRANTY CLAIMS

E. SETTLEMENT BENEFITS

Category 1: All Category 1 panels are eligible for removal.

Category 2: Category 2 panels are no longer eligible.

Please indicate a remediation preference from the options below. Installation of an Approved Inverter is not affected by remediation preference.

Option 1: Have an approved contractor selected by the Claims Administrator remove and replace the Class Panels by wattage, using panels and a contractor selected by the Claims Administrator.

Option 2: Have an approved contractor selected by the Claims Administrator remove and dispose of the Class Panels, and receive a cash payment of $2.35 per watt, less removal and disposal costs, which may be used to hire your own contractor to replace the Class Panels.

F. CERTIFICATION

I/we declare, under penalty of perjury under the laws of the United States of America, that the information in, and submitted with, this Claim Form is true and correct to the best of my/our knowledge and belief. I/we authorize the Claims Administrator to contact anyone else who may have supplied information or documentation about this claim, such as contractors, material suppliers, or declarants.
